Dental Claims
All dental claims will need to be submitted by your dental provider on an American Dental Association (ADA) claim form.
Please remember to submit a signed and dated authorization with all information provided.
You can assist your provider by presenting your Dental Plan ID card contained in your policy during your next visit. If you have questions, please contact an Aflac New York representative 1-800-366-3436.

- BENEFITS PAID REGARDLESS OF ANY OTHER INSURANCE – Aflac New York pays full policy benefits, unless assigned, regardless of other dental or medical coverage.
- IT’S FULLY PORTABLE – If you change jobs or retire, you can still keep the same benefits and premium rate you had as an employee. You may also purchase our Dental product directly!
- WELLNESS BENEFIT – Assists with the cost of routine care such as regular checkups and cleanings.
- OPTIONAL RIDERS – Optional dependent children orthodontic rider is available on payroll deduction.
